Now Enrolling for the 2022-23 School Year

REQUIREMENTS FOR KINDERGARTEN REGISTRATION

A copy of the following items must be turned in with the enrollment application:

• Birth Certificate (CHILD MUST BE 5 YRS OLD BY AUGUST 31)
• Immunization Records
• A photo ID of the parent or guardian
• Proof of Address (Current water, gas, or electric bill in Parent’s Name)
• **Health Assessment Form (must be completed by a Physician)
